Create or update your Clearinghouse Profile using TATY AGENCY LLC. Utilize the Clearinghouse Applicant Initiated (CHAI) System to initiate your fingerprint services screening and monitor progress.

At your appointment with TATY AGENCY LLC for fingerprint services, the provider will take fingerprints and a photograph, submitting them electronically to FDLE. Please bring the following materials: your ORI Number (specific to your profession), valid photo identification, and your Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN).

Before you leave your appointment for fingerprint services at TATY AGENCY LLC, make sure to obtain your Transaction Control Number (TCN) from the Livescan provider. You will need this number to track your screening status, as the Department cannot retrieve your TCN.

Volunteers submission to FDLE: $110.00 (Criminal History Check). For DCF/APD, the Criminal History/level II cost is $110.00 each for DCF and APD. The D.O.E.A. (Department of Elder Affairs) with Photo is $110.00, while all Chapter 493 Licenses (Department of Agriculture) are $110.00. D.B.P.R. (Department of Business & Professional Regulation) licenses are priced at $110.00. The A.H.C.A. (Agency for Health Care Administration) service costs $125.00 and includes a photo. Additionally, the Board of Pharmacy / Board of Nursing (NCLEX & CNA Exam) fee is $125.00 with photo. Medicaid with Photo also costs $125.00. For a Name Change (Clerk of Courts) the fee is $110.00, and Guardianship services are $110.00 as well. We charge $110.00 for all other licenses submitted electronically and $30.00 each for fingerprint card(s) FD-258. Notary services are available for $10.00, and MDCPS (Miami-Dade County Public Schools) services are $125.00.
